Medical Scholarship and Return Service (MSRS) Program for the AY 2023-2024
The scholarship is open for incoming first year and students of upper years.
Documentary requirements for application to the Medical Scholarship and Return Service (MSRS) Program:
- Proof of Filipino citizenship such as any government-issued documents showing proof of Filipino citizenship, including but not limited to certified true copy of Birth Certificate issued by the Local Civil Registry of Philippine Statistics Authority (PSA)/Philsys ID;
- Certificate of Good Moral Character:
- Additional requirements for priority groups (as applicable):
- Certification of Residency issued by the barangay;
- Certification as members of indigenous people or minority group/National Commission on Indigenous People (NCIP) certificate;
- Certification issued by the DOH-Bureau of Local Health Systems and Development that the applicant is a dependent of community health volunteer and
- Certification from Local Disaster Risk Reduction and Management Office (LDRRMO) or Department of Social Welfare and Development (DSWD) that the applicant is from calamity-prone and/or conflict areas.
- Latest Income Tax Return (ITR) of parents or guardian (not exceeding four hundred fifty thousand pesos) (Php 450,000.00). In the case of identified priority groups, Tax exemption or Tax Declaration from Bureau of Internal Revenue (BIR)/Social Case Study duly signed by a registered Social Worker where the applicant resides;
- Certificate of Acceptance from SUC/PHEI.
*Note: For Incoming First Year this will be issued by our college if the admission result comes out.
- Affidavit of no existing scholarship grant or with Return Service Obligation.
- If parent/s is/are suffering from chronic diseases (e.g. Undergoing Dialysis, Cancer patients/PWD) submit certification issued by the licensed physician. If Solo Parent, letter addressed to the Scholarship Committee indicating the circumstance of such status by the concerned parent.
Submit the requirements to com.scholarship.committee@wvsu.edu.ph and hard copies at the College Scholarship Office on or before May 31, 2023.
