Announcement for DOH PSSP and CHED Cash Grants for Incoming First Year – Academic Year 2020-2021

ANNOUNCEMENT

To all accepted incoming 1^st Year Medical Students:

Please prepare the following documents if interested to apply for:

Duly accomplished application form plus the required documents below:

REQUIREMENTS	DOCUMENTS TO BE SUBMITTED
Proof of Filipino Citizenship	Birth Certificate
Good moral character	Certificate from school and or barangay
Health condition ( including HBsAg screening test	Physically and Mentally Fit as certified by Gov,t Physician; Photocopy of laboratory results including HBsAg screening test
No existing scholarship grant	Affidavit of no existing scholarship grants
Passed the school admission test and other requirements	Certificate of acceptance of scholar
Low-income family	BIR Annual Filing of Tax of both parents/Guardian(s) or BIR Tax Exemption Certificate
Proof of Residency	Barangay Certificate
Priority Groups ( as applicable)
· Indigenous People or member of a minority group	National Commission on Indigenous People(NCIP) Certificate
· Dependents of Barangay Health Workers and other Gov’t employees	Certificate of Employment of Parent(s) / Guardian(s)
· Victim of Calamities	Certificate from Local Disaster Risk Reduction and Management Office (LDRRMO) or Department of Social Welfare and Development (DSWD)

Benefits of DOH Medical Scholarship

DOH shall grant to the Scholar a package that includes the following:

Tuition, laboratory, miscellaneous, and other school fees- to be paid directly to the partner school
Subsidy and Allowances which shall be paid directly to scholars:

Medical Subsidy for Post Graduate Internship/Within one year after Graduation

Allowances	Amount
Living Subsidy	P 4,500.00
Lodging Allowance	P 3,000.00
Transportation Allowance	P 880.00
One time Board Licensure/Examination Review and Application Fees	P 15,000.00

Annual Medical Insurance- PhP 3,600 per year or (P300/month), to be paid directly to the Philippine Health Insurance Corp.
