REQUIREMENTS (2 SETS):

• Résumé / Personal Data Sheet
• 2×2 ID picture, with white background
• Class Ranking
• 3 Letters of Recommendation
• 3 Character References
• NBI Clearance
• Certified true copies of the following documents:
          – West Visayas State University Medical Center
          – Certificate of Medical Internship
          – Board Rating
          – Residence Certificate
          – PRC ID (or receipt if not yet available)
          – BIR Registration / TIN (or receipt if not yet available)
